Procedure for ordering duplicates of higher education documents
The issuance of duplicates of education documents is carried out in accordance with the Procedure for ordering, manufacturing, issuing, and accounting for higher education documents and European-style diploma supplements, approved by the order of the Ministry of Education and Science of Ukraine dated 06.03.2015 No. 249 (as amended by the order of the Ministry of Education and Science of Ukraine dated November 02, 2020 No. 1351).
During the period of martial law, delays in the production of documents are possible.
Persons who have received an education document and its supplement upon completion of their studies at the university and have lost (damaged) one of these documents for valid reasons or discovered an error in them may submit (personally or through an authorized representative with a notarized power of attorney) a WRITTEN APPLICATION to the university for the production of their duplicates (a sample application is attached). https://kntu.kr.ua/file/content/18603/zrazok-zaiavy-na-zamovlennia-dublikativ-dokumentiv-pro-vyshchu-osvitu.pdf
The application can be written in office 305 or a scanned copy can be sent to the email address duplicatecntu@ukr.net or via phone number 066-396-60-58 (Viber, Telegram, WhatsApp).
ATTENTION!!! THE APPLICATION MUST BE WRITTEN BY HAND
The university will review your application and, if there are no objections, will begin the process of producing the duplicate.
After the duplicate is produced, you will be notified about the possibility of receiving it. You can receive the duplicate in person by presenting your passport or through an authorized representative.
The cost of producing a duplicate of an education document from October 1, 2025, is:

Diploma and diploma supplement – 270 UAH 00 kop. or 332 UAH 40 kop.

Diploma supplement separately – 228 UAH 60 kop. or 291 UAH 00 kop.
(the cost depends on the number of printed pages of the supplement)
Payment is made using the following details:

PAYMENT PURPOSE: For the production of duplicates of education documents. (it is mandatory to indicate the surname of the person ordering the duplicate for whom the original documents were issued.)

Порядок замовлення дублікатів документів про вищу освіту
Видача дублікатів документів про освіту проводиться відповідно до Порядку замовлення, виготовлення, видачі, обліку документів про вищу освіту та додатків до дипломів європейського зразка, затвердженого наказом Міністерства освіти і науки України 06.03.2015 р. № 249 (у редакції наказу Міністерства освіти і науки України від 02 листопада 2020 року № 1351).
На період воєнного стану можливі затримки із виготовленням документів.
Особи, які отримали документ про освіту і додаток до нього по завершенню навчання в університеті та з поважних причин втратили (пошкодили) один з цих документів або виявили в них помилку, можуть подати самостійно (або уповноважений представник, за наявності нотаріальної довіреності) ПИСЬМОВУ ЗАЯВУ до університету на виготовлення їх дублікатів (зразок написання заяви додається). https://kntu.kr.ua/file/content/18603/zrazok-zaiavy-na-zamovlennia-dublikativ-dokumentiv-pro-vyshchu-osvitu.pdf

УВАГА!!! ЗАЯВА ПИШЕТЬСЯ ВЛАСНОРУЧ
Університет розгляне Вашу заяву та, за відсутності зауважень, розпочне процес виготовлення дубліката.
Після виготовлення дубліката Вас повідомлять про можливість його отримання. Отримати дублікат можна особисто за пред’явленням паспорта або через уповноваженого представника.
Вартість виготовлення дубліката документа про освіту з 01.10.2025 року складає:

Диплом та додаток до диплома – 270 грн 00 коп. або 332 грн 40 коп.

Додаток до диплома окремо – 228 грн 60 коп. або 291 грн 00 коп.
(вартість залежить від кількості надрукованих сторінок додатка)
